Terms of Engagement for Holistic Fertility with Liz
Informed Consent to Nutritional Therapy with Liz Bortoli, BA Hons, PGCE, DipNT, mANP, mGNC
Consultations with a Nutritional Therapist includes taking a detailed case history; performing a physical exam if relevant, asking a lot of detailed questions, and following up on results and progression of your health. Nutritional Therapists employ a range of therapeutic techniques including dietary advice, lifestyle advice, recommended supplements and, where appropriate, functional testing.
I.What is Nutritional Therapy?
Nutritional therapy is the application of nutrition and lifestyle medicine sciences in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address imbalances and help support the body towards maintaining health.
Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach.

II.The Registered Nutritional Therapy Practitioner

Nutritional Therapy is practiced by non-medical professionals and is complimentary to other regulated forms of healthcare in England.

Nutritional advice will be tailored to support medically diagnosed conditions and/or health concerns agreed and identified by the two parties.

Nutritional Therapists are not permitted to diagnose or claim to treat medical conditions.

Nutritional Therapy is not a substitute for professional medical advice and treatment.

Nutritional Therapists may refer clients to a medical profession for testing and/ or any other concerns. We frequently work alongside medical professionals and will communicate with the consent of the client.

While the best course of action is continually sought for the patient, there always exists the possibility of side effects, adverse reactions or inefficacy of treatment. Liz Bortoli of ‘Holistic Fertility with Liz’ holds your safety and well-being as her top priority in the management of your case and welcomes all questions or concerns you may have.
I.I.I. The Registered Nutritional Therapy Practitioner requests that the Client notes the following:

The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy programme. There is no guarantee of a specific outcome.

Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.

Nutritional Therapists are not permitted to diagnose, or claim to treat, medical conditions.

Nutritional Therapist recommendations are not a substitute for professional medical advice and/or treatment.

Your Nutritional Therapist may recommend food supplements and/or functional testing as part of your nutritional therapy programme and may receive a commission on these products or services.

Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance. Liz Bortoli is a member of ANP – the Association of Naturopathic Practitioners.

This document only covers the practice of nutritional therapy within this consultation, and it will be made clear to you if there is an intention to provide services that are outside the scope of this practice.
I.V. The Client understands and agrees to the following:

You are responsible for contacting your GP or specialist about any health concerns you may have.

If you are receiving treatment from a GP, or any other medical or healthcare provider, you should tell him/her about your naturopathic nutrition protocol. This is necessary because of any possible reaction between medication and the nutritional programme.

Please also advise any other complementary medicine practitioners you are consulting.

It is important that you tell me/ your practitioner about any medical diagnosis you have received any prescription medication, herbal medicine or food supplements or over the counter medication you are taking as it may affect the naturopathic nutritional programme.

If you are unclear about any part of your plan then you should contact me/your practitioner immediately for clarification.

Your naturopathic nutritional programme and supplement plan will have a time frame and you should not continue with recommendations outside of this unless agreed by your naturopathic nutritional practitioner. This is to avoid any adverse reactions.

Please report any concerns about your programme to your nutritional practitioner for discussion at your next consultation.

You understand that the recommendations are personal to you and may not be appropriate for others.

Recording consultations using any form of electronic media is not allowed without the written permission of both practitioner and client.

It is the responsibility of the client to have a working internet connection for online appointments. If you are unable to connect as scheduled, the appointment is considered a ‘no show’ and charged in full.
V. Confidentiality and Data Protection
I, the Registered Nutritional Therapist Practitioner will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. Please refer to document ‘Privacy Policy’ for a detailed description of how this role is fulfilled.
